According to Indonesian Ministry of Heath Policy on Health Information System Stategy, much effort has been done to provide the information about public health services to community. Spatial characteristics are one of the major determinants in implementing services, even in the smaller municipal areas, and good quality spatial information is needed to inform decision making processes. However, limited funds, technical infrastructure and human resource capacity result in little or no access to spatial information for crucial infrastructure development decisions. The methodology that used to develop this system is Software Development Life Cycle (SDLC) / waterfall model. The tools Grass used to build a map and MapServer used to publish the digital map in the website. The result of this research is a Web based application research investigated whether it would be possible to develop a GIS for basic infrastructure planning and management using open source GIS. In this research, the information of public health services taken from Jogjakarta municipality prototype that can demostrate mapping the location of public health facilities that can be access from everywhere.
